WE DO NOT PARTICIPATE IN: MEDICAID, nor WORKER'S COMPENSATION. If you believe your problem is related to your work this is Worker's Compensation in which we DO NOT participate. REFERRAL AUTHORIZATIONS: Some insurance plans or specific insurance types require a formal authorization from the insurer to see the Allergy Specialist. This referral must be requested by the primary care physician. Such a referral must be in place prior to the visit. If it is not in place at such time it will be necessary to reschedule the appointment as most plans will not grant a referral after the visit. It is the responsibility of the patient (or parent/guardian) to obtain the referral. HIGH DEDUCTIBLE PLANS: Allergy services (including allergy injections) are NOT preventable health care and are subject to the deductibles of health insurance policies. If your plan deductible has not been met we may require payment for the deductible amount at time of visit. If you have such a plan, we suggest you speak to our staff in advance of the visit so you are completely informed of your potential obligations and responsibilities for the allergy services. CO-PAYS: If your plan has a fixed co-pay for a visit, the SPECIALIST co-pay applies. Similar to the policies of most doctor offices , this copay must be paid at the time of the visit. CO- INSURANCE Many plans have a co-insurance of anywheres from 10-25% for testing procedures in addition to the visit co-pay. If this amount is known in advance it may be requested at time of the visit. If the insurer notifies us of your coinsurance obligation when a claim is processed, you will be billed for the balance due. In some cases, allergy services are not a covered benefit of your policy even if this office particpates with that insurance company. In such case you will be responsible for that service. If you have questions about the benefits of your policy, kindly speak with our office about the procedures we do and ask your insurance carrier about the coverage of your policy. We do advise that any telephone statements by insurance company representatives be backed up by printed confirmation. CHILDREN'S COVERAGE IN DUAL HOUSEHOLD SITUATIONS: In situations where the child comes to the office with one parent (guardian) but due to divorce/separation or other circumstances another adult is financially responsible for their health care , we require the copays and coinsurances to be paid at the time of the visit by the adult who brings the child to the office. We need on file the names , addresses and phone numbers of both the adult who bringis the child and the adult financially responsible for their health care. We may request in advance that the other adult approve of the visit and agree to any potential financial obligations that may arise from the services provided. COLLECTION POLICY: This office accepts payment for obligations in cash, check or VISA or MASTERCARD (but no other types of credit cards). We do use the services of a national collection agency if necessary but try to avoid such through an understanding of obligations is advance of services and open communication between the patient (or parent/guardian) and our office. If there are special circumstances , please feel comfortable to discuss them with our office. |
